If your address has changed from what we have on file, you should send this change in to Charter HR.
The address correction should be submitted to us no later than Monday, January 16, 2012.
Taking care of your address change in a timely manner will prevent a delay in receiving your W-2.
This change should be submitted in writing through regular mail, e-mail, fax, or phone, giving us your full name, social security #, and date of birth.
